An app where users can submit their Health Declaration Form & COVID Report (Empty Data Source)
This app serves as a platform for its users to submit a Health Declaration Form whenever they would like to enter their organisation (workplace, school, construction site etc.), and their COVID-19 Test Results every fortnightly to keep track of their health status. The admins can then work with these data to monitor, manage and keep track of all the user's submission, ultimately dealing with giving or revoking access from the user to enter the organisation's premises
